When was the last time I truly SURPRISED myself? In fact, have I ever really managed to amaze myself before? <-that's good, but the "in fact" doesn't really fit.
I would replace "in fact" with "for that matter".

Both of them are fine, but the second option is much more suitable.

If I were the one writing it, I would say...
"When was the last time THAT I truly SURPRISED myself? In fact, have I ever really managed to amaze myself before?"

But either way is considered proper.

Either of the first two options would work perfectly, however to make the first option flow better I suggest writing it as "When was the last time I've truly surprised myself..." the reason is that, in this case, the contraction works better and, often times, people opt to use contractions in their speech i.e. can't instead of cannot.
